标题:Pulmonary arterial pressure influences the risk of occurrence of ventricular tachycardias and premature ventricular contractions

摘要:Background: Ventricular tachycardias (VTs) and premature ventricular contractions (PVCs) commonly arise from the right ventricular outflow tract (RVOT). The relationships between pulmonary arterial pressure (PAP) and VTs or PVCs arising from the RVOT (RVOT-VT and RVOT-PVC, respectively) are currently unknown. Objectives: The primary aim of this study was to determine the relationship between PAP and the occurrence of RVOT-VT and RVOT-PVC. Methods and Results: Eight hundred and two subjects without structural heart disease were enrolled consecutively between January 2009 and October 2010, and examined by echocardiography. The pulmonary arterial systolic pressure (PASP), pulmonary arterial diastolic pressure (PADP) and mean PAP (mPAP) were then estimated. Subjects were assigned to one of two groups: group A, comprising subjects diagnosed with RVOT-VT or RVOT-PVC; and group B, comprising subjects without any evidence of RVOT-VT or RVOT-PVC. Subjects in group A had a higher PASP than those in group B (23.21 ± 6.78 mmHg vs. 20.57 ± 4.69 mmHg; P = 0.014). Binary logistic regression analysis revealed that PASP was an important factor influencing the occurrence of RVOT-VT or RVOT-PVC (odds ratio, 1.20; 95% confidence interval, 1.01-3.50). Conclusions: Our findings suggest that an elevation of PASP, in a population without organic heart disease or other factors that can cause cardiac arrhythmia, is associated with the occurrence of RVOT-VT or RVOT-PVC.
